The Darwin centre is a place where the emphasis is on a person centred approach. It is an environment that is safe, comfortable and helpful for young people, their families friends and carers. Wider Active Support
We work alongside our partner organisations,RCPSYCH, undergoing QNIC accreditation. We work effectively with our community care teams to ensure a inclusive and consistent approach for young people and their families and carers. We have close relationships with our comissioners and provide regular updates and invitations to the involvement of the young persons care.
Co-Production
We listen to young people and monitor their stay within our environment, we reflect on the information the young person provides and discuss ways to improve in our processes and interaction with young people to ensure that their stay at the darwin centre has been as been a productive and positive experience.
Looking Back/Challenges Faced
The team consistence of health care professionals, admininstration staff, registered nurses, medical cover, and therapy professionals, we currently have vacancies and shortages in all of these roles. The vacancies seem to be constant with staff moving in and out which seems to be an ongoing challenge. The admirrable staff that we have fill in the shortages that we have through there passionate for working and helping young people and working together as a team.
Sustainability
Our recruitment process ensures that indiviuals are passionate about helping young people and their families, we assess monthly their involvement in care and encourage them to lead on the care and support of young people. We have regular staff meetings and team away days. We support trainning and participation for interest areas, we assess and monitor and supervise staff on a daily basis to ensure the highest levels of care for our patients.
Evaluation (Peer or Academic)
On admission we discuss expectations of the service with the young people, their families and carers. Throughout their stay we monitor whether this is being met or if their are any changes required. Prior to discharge we ensure that young person, families and carers complete and evaluation form. This information is recorded and feeds into a overall performance report for the darwin which monitors our progress and proformance.
Outcomes
The proformance report indicated in the previous answer, is used to monitor to our proformance and progress this is a excellent tool to identify the outcomes that are being met for your the yound person and their families.
Sharing
Here are the examples how the darwin centre shares and recieves information with simular services – invite partner service to the centre – visit regional conferences for networking – a weekly news letter loaded on our trust intranet -we have completed visits to our partnership centres -we attend local universities to share our practices and work
